Telephone Registrations
Call the number listed with the program, or call (614) 4-HEALTH (443-2584) or 1-800-837-7555. When you call to register by phone, you will be asked for a credit card number to cover any fees. You can phone anytime, 24 hours a day, seven days a week. Refunds/Cancellations
